4 Pound neonate in NICU is ordered Caffeine Citrate.
Caffeine Citrate 10mg/ml
20 mg/kg Bolus x 1, then 5 mg/kg/day
The Technician compounded the solution of caffeine citrate and filled the syringe for the bolus with 360 mg and the syringes next dose, 90 mg, to be placed in the unit dose cabinet for later delivery. The syringes were checked my the pharmacist and sent to the NICU. After the bolus dose the neonate was found to have tachycardia and started to seize. Lab tests determined that the neonate had high serum caffeine level. What should the bolus dose have been? How many millillters of the preparation should be drawn up?
The concentration of 10 g caffeine base and 10 g of citric acid are to be mixed with 1 L of sterile water. Remember to perform all of your conversions and calculate what the correct dose should have been for the bolus dose. ( What is the Bolus Dose?)
